A record 12,883 people in 21 states were sickened in the multistate Cyclospora outbreak that federal health officials declared over Friday, including at least 570 people who were hospitalized and two Michigan residents who died. The CDC update linked the illnesses to processed iceberg lettuce from Taylor Farms de Mexico and said recent infections had fallen sharply after a July recall. Yet the Food and Drug Administration has not determined how the parasite entered the product, leaving the central prevention question unresolved.

The distinction matters. Ending an outbreak means investigators no longer see continuing transmission from the implicated food and believe the recalled products are gone, not that every case has been counted or every failure has been identified. Symptoms began from June 14 through August 17, and the Centers for Disease Control and Prevention cautioned that cases can take six weeks or longer to reach federal surveillance systems.

The lettuce outbreak was also only part of an extraordinary national season. Since May 1, the CDC has received reports of 19,595 laboratory-confirmed infections acquired in the United States, compared with 1,180 during the comparable 2025 period. The agency’s case data list 1,043 hospitalizations across the broader 2026 count, meaning the ended outbreak produced nearly two-thirds of known domestic cases but not all of them.

The Recall Stopped Exposure Before the Case Count Stopped

Federal epidemiologic and traceback evidence converged on iceberg lettuce sourced from central Mexico and processed by Taylor Farms de Mexico. The company voluntarily recalled all iceberg lettuce from that source on July 17 after investigators connected the product to illnesses reported by customers of Taco Bell and to other retail and food-service channels. An FDA notice said the action covered specified retail products as well as lettuce distributed to the restaurant chain.

Most patients linked to the outbreak became ill before the recall. More than 1,000 infections began on a single day at the peak, according to the CDC, while August averaged fewer than two new onsets per day. The decline, the passage of every implicated best-by date and the removal of recalled lettuce from commerce supported the decision to close the public food-safety alert.

That sequence shows why an outbreak tally may continue climbing after a contaminated item disappears. People must first develop symptoms, seek care, receive the correct test and be interviewed about foods eaten days or weeks earlier. Health departments then compare epidemiologic, product-distribution and laboratory information. The final 12,883 count was 1,425 higher than the CDC’s August 27 update, even though most exposure had ended weeks before.

The Investigation Has Not Found the Contamination Route

The FDA investigation remains open. Inspectors completed on-site work and sample collection at growers and the processing facility in Mexico, but the agency said laboratory analysis was pending. Taylor Farms told Reuters that its central Mexico facility remained closed while government, company and independent investigations continued.

Cyclospora cayetanensis is a microscopic parasite transmitted when people consume food or water contaminated with human feces. Unlike many common intestinal pathogens, it generally must spend one to two weeks outside the body before becoming infectious, making direct person-to-person spread unlikely. Contamination can occur when produce contacts unsafe water or soil, but those general pathways do not establish what happened in this outbreak.

Fresh produce is especially difficult to investigate because it moves quickly through farms, processors, distributors, restaurants and retailers, often in combined products. A Post analysis estimated that at least 3.1 million pounds of lettuce were recalled. That scale expanded the number of purchase and dining histories investigators had to reconstruct while the perishable product itself was disappearing from kitchens and shelves.

The CDC used targeted genetic markers to group related Cyclospora samples, evidence that supported a common source. The technique is not the same as whole-genome sequencing routinely used for many bacterial outbreaks. Federal investigators have said stronger parasite-tracking tools would improve their ability to separate clusters and connect illnesses to foods, but better grouping alone cannot reveal whether contamination began in water, soil, equipment or another point in the supply chain.

A National Surge Extended Beyond One Supplier

The broader surveillance numbers prevent a misleading conclusion that the lettuce recall explains every 2026 infection. The 19,595 confirmed domestic cases were reported from 49 states and the District of Columbia, and at least 9,355 additional laboratory-confirmed cases still required investigation to determine whether they were acquired domestically or during travel. The CDC also knew of more than 6,100 other possible cases that were unconfirmed or awaiting interviews and submission.

Those categories should not be added together as though each were a confirmed domestic infection. They reflect different stages of surveillance, and the totals are preliminary. States may publish higher numbers because they use different definitions, include probable illnesses or update more frequently. The CDC outbreak count also includes people who traveled to one of the 21 affected states but lived elsewhere.

Before 2026, the highest U.S. seasonal total was about 4,700 cases in 2019, according to AP reporting. The new season therefore represents more than a statistical fluctuation. It indicates a much larger detection and disease burden, even though incomplete testing means the true number of infections was probably higher and unresolved clusters mean no single explanation accounts for the national increase.

Why the Clinical Burden Can Be Missed

Cyclosporiasis usually causes watery diarrhea, cramping, loss of appetite, fatigue, nausea and weight loss. Symptoms typically begin about a week after exposure but may appear from two days to two weeks or more later. Untreated illness can persist for more than a month, sometimes improving and then returning, according to the CDC’s clinical overview.

Diagnosis is not automatic. Routine stool testing may not look for Cyclospora, so clinicians may need to request a parasite-specific test and collect samples on more than one day. Most otherwise healthy people eventually recover, and antibiotics can treat the infection, but prolonged diarrhea can cause dehydration. Children, older adults and people with weakened immune systems face greater risk of severe illness.

Michigan reported the outbreak’s two deaths. The state health department said both people had significant underlying conditions that may have been affected by cyclosporiasis and dehydration, and both became ill before the recall. That careful wording in the state record describes contribution rather than asserting that the parasite alone caused each death.

Closing the Alert Does Not Close the Safety Question

Consumers no longer need to search refrigerators for the implicated lettuce because the recalled products have expired and are off the market. Ordinary produce washing remains important, but the CDC warns that washing cannot guarantee removal of Cyclospora. That limitation places much of the preventive burden upstream, where growers and processors manage agricultural water, worker hygiene, sanitation, traceability and supplier controls.

The FDA’s pending analyses may identify a specific contamination route, document contributing conditions or remain inconclusive. Each outcome would carry a different implication: a confirmed pathway could support targeted correction, while no definitive finding would leave regulators and industry relying on broader controls. The agency said it would continue working with U.S., Mexican, state and local partners and publish further updates as available.

The public-health emergency has receded, but the evidence does not support treating the episode as fully resolved. Federal authorities successfully connected a dispersed wave of illness to a perishable product and the exposures declined after the recall. What remains is the harder preventive task: explaining how contamination reached a supply chain large enough to produce the nation’s biggest recorded Cyclospora outbreak and whether the safeguards applied afterward can prevent another one.
