# U.S. Opens Formal Tesla Autopilot Investigation After 11 Emergency-Vehicle Crashes, Covering About 765,000 Cars

Federal auto-safety regulators have opened a formal investigation into Tesla’s Autopilot driver-assistance system after identifying 11 crashes since 2018 in which Teslas struck vehicles or equipment at emergency and first-responder scenes. The inquiry covers an estimated 765,000 Model S, Model X, Model 3 and Model Y vehicles from model years 2014 through 2021, according to contemporaneous reporting on the National Highway Traffic Safety Administration action.

The investigation puts one of the most heavily marketed advanced driver-assistance systems in the United States under a new level of federal scrutiny. A [Washington Post report](https://www.washingtonpost.com/business/2021/08/16/tesla-autopilot-investigation-nhtsa/?ref=theamericanquorum.com) said regulators identified 17 injuries and one death among the crashes under review, many occurring after dark where emergency lights, flares, illuminated arrow boards or road cones were present.

## Regulators focus on how Autopilot handles emergency scenes

The recurring pattern is what makes the inquiry significant. NHTSA is not examining a single isolated collision; it is investigating whether the system has a broader difficulty detecting or responding to stopped emergency vehicles and traffic-control scenes while Autopilot is engaged. Tesla describes Autopilot as a driver-assistance feature, not an autonomous-driving system, and tells drivers to remain attentive with hands on the wheel. But the system’s name and capabilities have long raised questions about whether drivers understand its limits. The National Transportation Safety Board, which investigates major crashes but cannot issue vehicle recalls, has repeatedly pressed for stronger safeguards around partially automated driving. ## A broader federal data push is already underway

The Autopilot investigation arrives less than two months after NHTSA ordered manufacturers and operators to report certain crashes involving advanced driver-assistance systems and automated-driving systems. The agency’s June 29 [standing general order](https://www.nhtsa.gov/press-releases/nhtsa-orders-crash-reporting-vehicles-equipped-advanced-driver-assistance-systems?ref=theamericanquorum.com) requires companies to submit crash information on an expedited timetable, giving regulators a more systematic way to identify safety patterns that might otherwise remain scattered across police reports, company databases and media accounts.

That order reflects a shift in the federal approach to increasingly automated vehicles. Instead of waiting for a fully autonomous future, regulators are focusing on the systems already operating on public roads and the ways human drivers interact with them. Partially automated features can keep a car centered in a lane, adjust speed and react to traffic, but they still depend on drivers to recognize situations the software cannot handle.

Those conditions create a demanding perception problem: the system must interpret stationary vehicles, lane closures, flashing lights and altered traffic patterns that differ from ordinary highway driving.

## The investigation tests Tesla’s approach to driver supervision

One central issue will be whether Tesla’s steering-wheel-based driver monitoring is sufficient. Many competing systems increasingly use driver-facing cameras to confirm that a motorist’s eyes remain on the road. Tesla vehicles include cabin cameras on some models, but Autopilot has historically relied heavily on steering-wheel torque to detect whether the driver is engaged.

That distinction matters because a driver can satisfy a steering-wheel prompt without necessarily maintaining continuous visual attention. The investigation could therefore have consequences beyond the specific emergency-vehicle crashes if regulators conclude that stronger monitoring is necessary for systems that perform sustained steering and speed control.

## Autopilot remains legal to use while the inquiry proceeds

Opening a preliminary evaluation does not mean NHTSA has determined that Autopilot is defective, and the agency has not ordered Tesla to disable the feature. Regulators will gather data, examine crashes, evaluate the system and decide whether the evidence warrants a deeper engineering analysis or a recall.

For Tesla, the inquiry reaches beyond one software feature. The company has made automated-driving capability central to its technology identity and sells an additional package called Full Self-Driving Capability, which despite its name also requires active driver supervision. Any federal finding about how Tesla designs, labels or monitors Autopilot could influence expectations for those more ambitious systems.
