Pennsylvania has recorded four measles-associated deaths in 2026 after confirming two additional deaths Tuesday, as the state’s outbreak reached 693 cases across 38 counties.

The Pennsylvania Department of Health said the newly confirmed deaths involved unvaccinated residents of Jefferson and Mifflin counties. Its [investigations](https://www.pa.gov/agencies/health/newsroom/doh-confirms-two-additional-measles-associated-deaths-?ref=theamericanquorum.com), conducted with county coroners, found clinical evidence of infection, positive laboratory tests and no unrelated cause of death. The two deaths followed two measles-associated deaths reported in Lancaster County in August, the state’s first in 35 years.

The development turns a sustained outbreak into a sharper warning about the consequences of widening immunity gaps. Pennsylvania’s total represents more than one-fifth of the 3,294 confirmed U.S. cases reported to federal officials through Sept. 10\. The state’s case count, hospitalization burden and geographic spread show how quickly measles can move once it reaches communities where too few people are protected.

## Four deaths clarify the outbreak’s stakes

Pennsylvania uses the term “measles-associated” deliberately. The state said its reviews confirmed measles infection and ruled out unrelated causes, but that formulation does not imply that every death followed the same clinical pathway. The [Associated Press](https://apnews.com/article/measles-deaths-pennsylvania-5267e2deff65b12941e6a1c279976f13?ref=theamericanquorum.com), citing the Mifflin County coroner, reported that an 18-year-old died from acute disseminated encephalomyelitis, a rare neurological complication. A 40-year-old Jefferson County woman and two infants in Lancaster County accounted for the other deaths.

None of the four people had been vaccinated, according to state officials, although the infants were too young to receive the routine first dose. That distinction matters: infants and some people with medical contraindications depend partly on high community immunity rather than their own vaccination. Measles can also cause pneumonia and brain swelling, and death remains rare but documented even in health systems with modern supportive care.

The state’s [latest update](https://www.pa.gov/agencies/health/newsroom/pa-department-of-health-provides-statewide-update-on-measles-out2?ref=theamericanquorum.com) reported 133 hospitalizations among 693 cases, or roughly one hospitalization for every five infections. Only four cases were recorded among vaccinated people. Those figures do not prove that every hospitalization was caused by a severe complication, because patients may also be admitted for isolation, but they demonstrate a substantial operational burden on hospitals and public-health teams.

## Pennsylvania mirrors a national resurgence

The outbreak is part of the worst U.S. measles year in decades. The Centers for Disease Control and Prevention counted [3,294 cases](https://www.cdc.gov/measles/data-research/index.html?ref=theamericanquorum.com) through Sept. 10, up from 2,289 during all of 2025 and 285 in 2024\. Ninety-five percent of this year’s cases were linked to outbreaks, rather than isolated infections, indicating that transmission is being sustained after the virus enters susceptible communities.

CDC data also show the national measles, mumps and rubella vaccination rate among kindergartners fell from 95.2% in the 2019–2020 school year to 92.4% in 2025–2026\. That left an estimated 280,000 kindergartners at risk. National averages can conceal larger local gaps, and measles tends to spread most efficiently in clusters of unvaccinated people even when statewide coverage appears comparatively high.

The United States declared measles eliminated in 2000, meaning continuous domestic transmission had stopped, not that imported cases had disappeared. International travel continues to reintroduce the virus. The public-health test is whether vaccination and rapid case investigation can prevent those introductions from becoming long transmission chains. International officials are expected to review whether the United States has retained elimination status later this year.

## Immunity gaps make containment difficult

Measles spreads through the air and can infect susceptible people after an ill person has left a room. That high transmissibility makes contact tracing, isolation and post-exposure vaccination time-sensitive. It also means a modest decline in statewide coverage can have an outsized effect if the unvaccinated population is concentrated in particular schools, neighborhoods or social networks.

Two doses of the MMR vaccine are about 97% effective at preventing measles, while one dose is about 93% effective, according to the [CDC](https://www.cdc.gov/measles/about/index.html?ref=theamericanquorum.com). No vaccine eliminates every breakthrough infection, but Pennsylvania’s outbreak data show that fewer than 1% of reported cases occurred in vaccinated people. The current evidence therefore points to inadequate coverage, not vaccine failure, as the central vulnerability.

Containing the outbreak requires more than making doses available. Health departments must identify exposures, notify contacts, arrange testing that does not expose waiting rooms, and provide credible information to families. Hospitals and clinics also need screening protocols that recognize fever, cough and rash before an infectious patient enters shared clinical space.

## Public confidence is weakening

The medical response is unfolding while confidence in childhood vaccination declines. A national [Reuters/Ipsos poll](https://www.reuters.com/business/healthcare-pharmaceuticals/trust-measles-vaccines-falls-us-reuters-ipsos-poll-finds-2026-09-15/?ref=theamericanquorum.com) completed Monday found that 76% of U.S. adults considered measles, mumps and rubella vaccines safe for children, down from 84% in May 2020\. The survey included 1,143 adults and had a three-percentage-point margin of error.

A bipartisan majority still supported vaccination, but the decline complicates outbreak control. Public skepticism can delay routine doses, reduce participation in emergency clinics and make communities more receptive to unsupported claims. Access barriers, missed well-child visits and exemptions also contribute to lower coverage, so communication alone cannot close every gap.

Pennsylvania’s four deaths establish the human cost of the outbreak without resolving every dispute over national vaccine policy or death classification. What the evidence does establish is narrower and consequential: confirmed infections are rising, hospitalizations are substantial, the deaths occurred among unvaccinated people, and two-dose vaccination remains highly protective. The next indicators will be whether weekly cases and hospitalizations fall, whether local coverage improves, and whether transmission ends soon enough to preserve the country’s measles-elimination status.
