# U.S. Vaccination Push Reaches Adolescents as More Than 60% of Adults Receive at Least One COVID-19 Shot

The United States entered a new phase of the COVID-19 vaccination campaign this week as adolescents began receiving the Pfizer-BioNTech vaccine and more than 60% of adults had received at least one dose, even as the pace of first vaccinations continued to slow.

The expansion to 12- through 15-year-olds follows the Food and Drug Administration’s decision to broaden emergency use of the Pfizer-BioNTech vaccine and the Centers for Disease Control and Prevention’s recommendation that providers begin vaccinating adolescents immediately. Pfizer and BioNTech said the authorization was supported by a trial of 2,260 participants in which the vaccine showed [100% observed efficacy](https://www.pfizer.com/news/press-release/press-release-detail/pfizer-and-biontech-receive-first-us-authorization?ref=theamericanquorum.com) against symptomatic COVID-19 in the study population, although the confidence interval around that estimate remains important because relatively few cases occurred.

At the same time, national indicators are moving in a favorable direction. CDC’s [weekly review](https://archive.cdc.gov/www%5Fcdc%5Fgov/coronavirus/2019-ncov/covid-data/covidview/past-reports/05212021.html?ref=theamericanquorum.com) reported that cases, hospitalizations and deaths are declining. As of May 20, 60.5% of U.S. adults had received at least one dose and 48.4% were fully vaccinated.

## Adolescents become the newest eligible population

The CDC director formally adopted the recommendation of the Advisory Committee on Immunization Practices after the panel reviewed evidence on safety, immune response and efficacy. In a [May 12 statement](https://archive.cdc.gov/www%5Fcdc%5Fgov/media/releases/2021/s0512-advisory-committee-signing.html?ref=theamericanquorum.com), CDC said providers could begin vaccinating people ages 12 to 15 immediately.

The agency’s detailed [MMWR recommendation](https://www.cdc.gov/mmwr/volumes/70/wr/mm7020e1.htm?ref=theamericanquorum.com), published this week, describes the Pfizer-BioNTech vaccine as a two-dose series administered three weeks apart. The report said the trial produced no symptomatic cases among vaccinated adolescents compared with cases in the placebo group during the analyzed period, yielding an efficacy estimate of 100% with a 95% confidence interval of 75.3% to 100%.

CDC’s separate [GRADE review](https://www.cdc.gov/acip/grade/covid-19-pfizer-biontech-vaccine-12-15-years.html?ref=theamericanquorum.com) classified the certainty of evidence for prevention of symptomatic COVID-19 as high. The committee also evaluated reactogenicity, serious adverse events and the broader public-health consequences of vaccinating adolescents.

The [evidence-to-recommendation framework](https://www.cdc.gov/acip/evidence-to-recommendations/covid-19-pfizer-biontech-etr-12-15-years.html?ref=theamericanquorum.com) emphasized that adolescents account for a meaningful share of infections, can transmit the virus within households and, although less likely than older adults to develop severe disease, can still be hospitalized or develop complications such as multisystem inflammatory syndrome in children.

## The national campaign is succeeding — unevenly

The expansion comes as the adult vaccination campaign crosses an important threshold. CDC data show that a majority of adults have received at least one dose, with coverage far higher among older adults than younger ones. By May 22, a CDC analysis found that 57% of all adults had initiated vaccination, including 80% of adults 65 and older but only 38.3% of adults ages 18 to 29\. The agency’s [coverage analysis](https://www.cdc.gov/mmwr/volumes/70/wr/mm7025e1.htm?ref=theamericanquorum.com) also documented slower uptake in less urban counties and communities with greater social vulnerability.

Those gaps are becoming more important as supply constraints ease. Earlier in the campaign, the central problem was obtaining enough doses and scheduling scarce appointments. Increasingly, the challenge is persuading and reaching people who have not yet been vaccinated, including younger adults, rural residents and communities facing logistical barriers.

The addition of adolescents may help families and schools plan for the next academic year, but it also complicates delivery. Pediatricians, family physicians, pharmacies and school-linked programs will all play roles in reaching younger patients. Unlike adult vaccination, decisions for minors usually require parental involvement, making communication about safety and benefits especially important.

## Falling cases create optimism but not uniform risk

The improving epidemic curve is visible in multiple measures. CDC’s May 21 review reported declines in reported cases, emergency department visits, hospital admissions and deaths. The improvement is occurring as vaccination rises and after the severe winter surge that overwhelmed hospitals in many regions.

But the national average can obscure substantial local variation. Communities with lower vaccination coverage remain more vulnerable to outbreaks, and variants continue to circulate. Public-health officials are therefore trying to balance a message of progress with warnings that vaccination gaps can preserve pockets of transmission.

The adolescent authorization also changes the mathematics of community protection. Millions of people who were previously ineligible can now be immunized, reducing the share of the population that must remain susceptible simply because of age restrictions. It does not eliminate the need for adult vaccination, but it broadens the population capable of contributing to lower transmission.

## The next challenge is demand

Pfizer and BioNTech had already reported [topline adolescent results](https://www.pfizer.com/news/press-release/press-release-detail/pfizer-biontech-announce-positive-topline-results-pivotal?ref=theamericanquorum.com) in March and formally [requested expanded authorization](https://www.pfizer.com/news/announcements/pfizer-and-biontech-request-regulatory-agencies-expand-emergency-use-their-covid?ref=theamericanquorum.com) in April. The rapid sequence from trial results to authorization reflects the urgency of the pandemic but also places unusual attention on post-authorization surveillance as millions of younger people become eligible.

For the broader campaign, however, the central policy issue is shifting. The country is no longer primarily constrained by the number of doses on hand. It is increasingly constrained by willingness, convenience and confidence.

That means the next phase will be more decentralized than the first. Mass vaccination sites and hospital systems remain important, but local clinicians, pharmacies, employers, schools and community organizations will have to reach people who did not rush to schedule appointments when eligibility opened.

The numbers this week provide the strongest evidence yet that vaccination is helping push the epidemic downward. They also show why progress cannot be measured by a single national percentage. A campaign that has protected most older adults is now moving toward younger people, including adolescents, and the success of that transition will help determine whether falling cases become a durable summer trend.
