# Texas Dairy Worker Tests Positive for H5N1 as U.S. Confirms First Human Bird-Flu Case Linked to Infected Cattle

AUSTIN, Texas — A Texas dairy worker has tested positive for highly pathogenic H5N1 avian influenza after exposure to cattle believed to be infected, marking the first known human case associated with the emerging outbreak in U.S. dairy herds. The patient developed conjunctivitis as the only reported symptom and is being treated with the antiviral oseltamivir, according to the Centers for Disease Control and Prevention's April 1 [announcement](https://www.cdc.gov/media/releases/2024/p0401-avian-flu.html?ref=theamericanquorum.com). Texas health officials separately confirmed the case and said the person had direct exposure to dairy cattle, while emphasizing that the risk to the general public remains low. The Texas Department of State Health Services' [notice](https://www.dshs.texas.gov/news-alerts/dshs-reports-first-human-case-avian-influenza-texas?ref=theamericanquorum.com) said there is no evidence of sustained person-to-person transmission.

The human infection comes one week after federal and state veterinary agencies confirmed H5N1 in dairy cattle in Texas and Kansas, an unusual development because the virus has historically been associated mainly with wild birds and poultry. ## A cattle outbreak changes the surveillance problem

H5N1 has circulated widely in wild birds since 2022 and has caused devastating outbreaks in commercial poultry flocks. Sporadic infections have also occurred in mammals, usually after exposure to infected birds. Sustained transmission in dairy cattle, if confirmed, creates a different epidemiological challenge because cattle are handled closely by workers and move between farms and states through routine agricultural commerce.

USDA confirmed additional infected herds during the week, including in New Mexico, and said epidemiological work was underway to understand transmission routes. Investigators are examining movement of cattle, shared equipment and other farm contacts while testing animals with reduced appetite and milk production.

## The patient's illness is mild, but the event matters

The Texas patient reportedly experienced redness and irritation of the eye rather than respiratory disease. Conjunctivitis can occur when influenza virus contacts ocular tissue, particularly in occupational settings involving close exposure to infected animals. The person was instructed to isolate and received antiviral therapy. Public-health officials are monitoring close contacts for symptoms.

The mild presentation is reassuring for the individual patient but does not eliminate the broader significance. Each mammalian infection gives the virus another opportunity to replicate in a biological environment different from birds. Influenza viruses continually mutate, and public-health agencies watch for changes that could improve their ability to bind human respiratory receptors or transmit efficiently between people. As of Saturday, there is no evidence that the dairy-associated virus has acquired sustained human-to-human transmission.

## Food-supply risk and worker risk are different questions

Federal agencies are drawing a sharp distinction between occupational exposure on farms and risk from commercially sold dairy products. Milk from visibly sick cattle is supposed to be excluded from the food supply, and pasteurization is designed to inactivate pathogens. FDA and USDA say the existing safeguards make the commercial milk supply safe. The immediate concern is therefore concentrated among people who work directly with infected or potentially infected animals, raw milk, bodily fluids and contaminated surfaces.

Texas issued a more detailed [health alert](https://www.dshs.texas.gov/news-alerts/first-case-novel-influenza-h5n1-texas-march-2024?ref=theamericanquorum.com) advising clinicians to consider H5N1 in people with compatible symptoms and exposure to potentially infected animals. Farm workers are being urged to use protective equipment, including gloves, eye protection and appropriate respiratory protection when handling sick cattle or materials that may be contaminated.

## The response depends on fast testing and sequence analysis

CDC laboratories are analyzing viral sequences from the patient and animal cases for markers associated with adaptation to mammals or antiviral resistance. State animal-health agencies are tracing herd movements, while USDA is expanding diagnostic testing. The current evidence does not support broad public restrictions or changes in consumption of pasteurized dairy products. It does support intensified surveillance among cattle, farm workers and people exposed to raw animal products. The critical questions are how widely the virus is circulating in dairy herds, whether cattle-to-cattle transmission is occurring efficiently and whether additional human infections appear.

As of Saturday, federal officials continue to rate the immediate public risk as low. But the appearance of H5N1 in dairy cattle and a directly exposed worker creates a new interface between animal and human health. The outbreak is now not only an agricultural problem but a test of whether U.S. surveillance can detect and contain cross-species influenza transmission before it becomes a larger human threat.
