# U.S. Returns to 100,000 Daily COVID-19 Cases as Delta Pushes Hospitalizations Higher in Low-Vaccination States

The United States is again averaging more than 100,000 new COVID-19 infections a day, a level last seen during the winter surge, after cases multiplied nearly tenfold from their late-June low under the pressure of the highly transmissible Delta variant. More than 44,000 Americans are currently hospitalized with the disease, up about 30 percent in a week and nearly four times the June level, according to figures reported Saturday in an [Associated Press account](https://www.pbs.org/newshour/nation/us-now-averaging-100000-new-covid-19-infections-a-day?ref=theamericanquorum.com).

The resurgence is occurring despite a vaccination campaign that has now given at least one dose to 70 percent of U.S. adults. That apparent contradiction is explained largely by geography: vaccination rates vary sharply, and the heaviest burden of cases and hospital admissions is concentrating in states and communities where fewer people are protected.

## A six-week reversal sends cases from 11,000 to more than 100,000

Daily infections bottomed out at roughly 11,000 in late June, after months of vaccination and declining transmission. Six weeks later, the seven-day average has risen to about 107,000\. The speed of that reversal illustrates the advantage Delta has over earlier versions of the virus and the amount of susceptible population that remains available to infect.

The CDC's August 6 [weekly review](https://stacks.cdc.gov/view/cdc/108673?ref=theamericanquorum.com) says cases, hospitalizations and deaths are increasing again in nearly all states and that the greatest spread and most severe outcomes are occurring in places with lower vaccination rates. The agency continues to describe vaccination as the most effective prevention measure, even as it acknowledges that breakthrough infections can occur.

The national numbers also hide sharper local strain. The [Guardian reported](https://www.theguardian.com/world/2021/aug/06/us-covid-coronavirus-delta-variant?ref=theamericanquorum.com) Friday that the seven-day average of hospital admissions had increased more than 40 percent from the previous week, with hospitals in several Southern states again facing shortages of intensive-care capacity and staff.

## Seven states are producing about half of new cases and hospitalizations

The White House COVID-19 response team said Thursday that Florida, Texas, Missouri, Arkansas, Louisiana, Alabama and Mississippi accounted for roughly half of new cases and hospitalizations during the previous week even though they contain less than one quarter of the U.S. population. Florida and Texas alone accounted for about one-third of both measures, according to the [August 5 briefing](https://www.presidency.ucsb.edu/documents/press-briefing-white-house-covid-19-response-team-and-public-health-officials-50?ref=theamericanquorum.com).

The concentration is a reminder that the country is not experiencing one uniform epidemic. Highly vaccinated communities can still see infections, but the pressure on hospitals is much greater where a large share of adults remain unvaccinated. Because intensive-care beds, nurses and emergency departments are local resources, a regional surge can become a health-system crisis even when national capacity appears adequate.

President Biden made the same geographic argument in [remarks](https://www.presidency.ucsb.edu/documents/remarks-the-covid-19-response-and-national-vaccination-efforts-and-exchange-with-8?ref=theamericanquorum.com) Tuesday, saying 165 million Americans were fully vaccinated while about 90 million eligible people had not received a first dose. He said the lowest-vaccination states were recording case rates many times higher than the most vaccinated states.

## The vaccination milestone is real, but incomplete

On Monday, the country reached a goal the administration had originally hoped to meet by July 4: 70 percent of adults had received at least one vaccine dose. A contemporaneous [CBS report](https://www.cbsnews.com/news/covid-19-vaccine-70-percent-adults-goal-white-house/?ref=theamericanquorum.com) put the fully vaccinated share of adults at just over 60 percent, leaving tens of millions of eligible people without complete protection.

Vaccination rates are beginning to rise again in some states hit hardest by Delta. That is encouraging, but the effect is delayed. People receiving a first dose today are not immediately fully protected, particularly if they receive a two-dose vaccine. The virus can therefore continue exploiting current gaps for several weeks even if vaccination accelerates now.

The vaccines remain most important for preventing the outcomes that matter most. Hospitalizations and deaths have increased but remain well below winter peaks relative to the number of cases, largely because older adults—the group at greatest risk—have high vaccination coverage. That difference is why federal officials are resisting the idea that the country must return to the broad shutdowns of 2020.

## Breakthrough infections complicate the message without erasing protection

Delta has made the communication challenge harder because fully vaccinated people can still become infected and, in some cases, transmit the virus. A CDC [investigation](https://www.cdc.gov/mmwr/volumes/70/wr/mm7031e2.htm?ref=theamericanquorum.com) posted July 30 found 469 cases associated with large gatherings in Barnstable County, Massachusetts; 74 percent occurred in fully vaccinated people, and Delta was identified in about 90 percent of sequenced specimens.

The study found similar PCR cycle-threshold values in vaccinated and not-fully-vaccinated cases, raising concern that some vaccinated people with Delta breakthrough infections can carry enough virus to transmit it. But the outbreak does not show that vaccines no longer work. Only five people in the Massachusetts cluster were hospitalized, and no deaths had been reported at the time of the study.

That distinction is now central to public-health messaging: vaccination greatly lowers the risk that COVID-19 becomes severe, while Delta makes it less realistic to promise that vaccination will prevent every infection. The CDC has therefore restored indoor mask recommendations in areas of substantial or high transmission as an additional layer of protection.

## Hospitals, not case counts alone, will determine the severity of this wave

Saturday's return to a six-figure daily case average is psychologically significant, but the most important question is how many infections convert into serious illness. During the January peak, more than 120,000 Americans were hospitalized at once. The current total is far lower, but it is rising fast enough to strain specific regions.

Health officials in Houston, Missouri and parts of the South have reported difficulty finding beds and moving patients between facilities. The AP's [national survey](https://www.pbs.org/newshour/nation/us-now-averaging-100000-new-covid-19-infections-a-day?ref=theamericanquorum.com) describes ambulances waiting to offload patients and states preparing additional transport capacity when local hospitals fill.

That is the near-term danger of a geographically concentrated epidemic. A vaccinated majority can keep national death rates below previous peaks while an unvaccinated minority is large enough to overwhelm hospitals in particular communities.

The next several weeks will show whether renewed vaccination, masking and voluntary behavior changes can slow Delta before hospital admissions climb much further. The country has tools it did not possess during earlier surges—hundreds of millions of vaccine doses, far more knowledge about treatment and a surveillance system tracking variants in real time. But Delta is exposing the limits of those tools when they are unevenly used.

The United States has reached two milestones in the same week: 70 percent of adults with at least one vaccine dose and more than 100,000 average daily cases. Together, they define the central problem of the current phase. National vaccination progress is substantial, but the remaining pockets of low coverage are large enough for a faster virus to rebuild an epidemic.
